If you meet our grant requirements and your organization is a match to our funding interests and priorities, complete the form to the right and attach a MS Word document that includes the following information:

2. Does the Organization have an IRS 501(c)(3) tax designation?  

3. What is your Organization's mission statement? 

4. Title and target timeline for project requesting funding? 

5. Executive summary of your Project?

6. Describe your project's alignment with our Foundation’s mission. 

7. When do you need the funds? 

8. Requested amount? 

9. Total project budget?

10. How will you define success for this project? 

11. Who, (name and title), will be responsible for this project?
